How to handle requests for lost or replacement tax forms
- Employee: Call to State Accounting Payroll team for next steps. State Accounting Payroll verifies information and issues replacement as appropriate. IOC may also send email on behalf of visitor.
- Retiree: Direct retirees to their retirement system to get a copy or to email w2request@illinoiscomptroller.gov email address. IOC may also send email on behalf of visitor.
- Vendor: Employee will either direct to https://illinoiscomptroller.gov/vendor-services/vendor-1099-forms or direct them to Vendor Unit at 217/785-4422.
